As a courtesy to you, we will bill your insurance company* directly. A statement will be sent to you after we receive your insurance company's determination to the amount they will cover. Your insurance plan may require payment of a co-pay and/or deductible. You will be asked to pay the co-pay and/or deductible at the time you receive service to help us keep billing costs down. If your insurance company has not responded to our claim, or your insurance company decides not to cover a service provided within 120 days, you will be responsible for paying any balance in full.

• Unicare
• All HMOs
